The Community Health Connector (CHC) Program is part of a broader, comprehensive initiative within MCFS to address inequalities, racial disparities, and service access in community health and other social serving systems for low-income immigrant seniors.

CHC Program provides services to:

  • Homes

  • Communities

  • Adult Day Programs

  • Hospitals

  • Nursing Homes

CHC Program services seniors, adults, children, intellectually disAbled, and physically disAbled. We offered the following services to our participants and their families:

  • Companion Service

  • Respite

  • In-home and Community Supports

  • Community Participation Supports

  • Job Search / Job Finding

  • Homemaker / Chores

  • Medication Reminder

  • Meal Preparation

  • Grocery Shopping / Errands

  • Laundry
